Launch of The Support Loop

The North Metro and West Metro Active Service Model (ASM) & Diversity Team are pleased to announce the launch of a new ASM resource The Support Loop.

The development of this new resource demonstrates sector engagement and partnership with over 30 Home and Community Care (HACC) funded organisations from around the State actively involved in its development. Managers, Team Leaders and Community Care Workers from the participating organisations contributed their ideas, enthusiasm and commitment to this project.

The Support Loop resource has been designed to provide a feedback and monitoring framework to support organisations who have responsibility for HACC services delivered in the home or in a social support setting including home care, social support, home maintenance, transport and personal care.

An initiative of the North Metro and West Metro Region ASM Industry Consultants, The Support Loop recognises the vital role of Community Support Workers.  Community Support Workers support people to meet their support plan goals whilst monitoring progress and providing feedback.   Effective monitoring and feedback ensures that support plans are appropriate, relevant and responsive to the changing needs and circumstances of the person.

The Support Loop resource has been developed to provide HACC funded organisations with ideas and approaches for assessing their current practice and provides opportunities for improving monitoring and feedback communication systems.

This resource has been prepared prior to the transition of the HACC program to the Commonwealth Home Support Program, however it has been developed based on a philosophy of wellness that builds on the strengths, capacity and goals of people.

The state wide ASM Industry Consultants have contributed to the development of this resource.  Details of a training and implementation approach to using The Support Loop will be available in the coming months.

The Hume Whittlesea Primary Care Partnership (HWPCP) and the ASM ICs will distribute one copy of The Support Loop to all HACC funded organisations in Victoria. In the meantime a copy of The Support Loop can be downloaded from the HWPCP website link
